Guide to Filling Up Consignment Note

To send a delivery with D3 Express, you will need to prepare a Local Courier Consignment note.

1. It tells D3 Express where your packages is going, what service you require and how you intend to pay.
2. It give you proof of consignment with a unique number which we can track for billing or delivery if needed.
3. It give us your instructions and authority to collect or deliver on your behalf.

All you have to do is

• Fill up the collection and delivery details in full (compulsory)
• Indicate the weight and item description if you prefer (not compulsory)
• Place your signature in the sender column and date. (compulsory)
• Tick the type of service you need (not compulsory)
• Fill in the bottom right hand corner for your special instructions for collecting anything upon delivery.(compulsory)
• Ensure that the D3 Express confirms the receipt of your packages by initialling on the bottom left hand corner. Please retain the carbonized copy (red-sender, blue-recipient, white-D3) (compulsory)
